Denial of claim is the refusal of an insurance company or carrier to honor a request by an individual (or his or her provider) to pay for health care services obtained from a health care professional.
  • Common Reasons for Insurance Claim Denials. Insurance companies will sometimes deny seemingly legitimate claims. Some of the reasons an insurer might deny a claim include: Claim was not filed timely. Incident occurred outside of coverage dates. Policy exclusions apply. Policyholder breached a provision of the contract.
  • Some reasons why life insurance is denied: 1) You applied for a policy for which you do not qualify due to a medical condition, being overweight or underweight per the policy guidelines, or are out of the age limits of the policy. 2) You are not a US citizen or do not have a green card or work visa.
